Expression of Interest: Online Clinical Support Tools for General Practice

South Western Melbourne Medicare Local (SWMML) is pleased to announce that we have secured the following Online Clinical Support Tools for General Practices in Wyndham and Hobsons Bay. We are inviting EOI from those GPs who wish to have access to the following:

·         Map of Medicine internationally acclaimed care pathways solution which gives you the power to improve the decision making and referral quality at the point of care by following localised care pathways specially developed for your region which are accurate, locally relevant and endorsed by the world’s leading medical bodies. In order to localise international partnerships, we are looking to establish working groups for specific health needs with remuneration available.

·         Topbar is designed to provide decision-support at the point of care by delivering useful patient-specific alerts and prompts to all health providers about their patients on the day. Topbar is comprised of individual apps which allow the health providers to stay focused on the patient specific needs and assists your health service in achieving this important task.

Ø  MBS Prompts provides proactive real-time prompts focusing on the completion and claiming of MBS items

Ø  Patient Health Summary (PHS) provides key clinical and demographic summary information about your patient.

Chronic Disease Management Applications

·         PAT CAT Patient Aggregation Tool (PAT) by Pen Computer System is also coming soon. It aggregates data from multiple sources including de-identified Classic Clinical Audit Tools (CAT) data files from General Practices. Practices will be able to benchmark their de-identified practice data against other aggregated practice data from across their region. In addition PAT CAT creates graphs, reports and population health maps.
