Practice support 

We provide support to general practice and allied health including Clinicians, Practice Managers, Practice Nurses and adminstrative staff in the Hobsons Bay and Wyndham local government areas.

SWMML provides support in areas of practice accreditation, cold chain audits, eHealth readiness, referral pathways, training and education workshops and networking events to support collaboration with GPs and Allied Health Professionals and support staff.

In addition we host reference or advisory groups for the clinician led input into service programs.

Nurse Leadership Program

The role of the practice nurse in the practice environment and in the community requires ongoing training and support. As part of the Nurse Leadership Project three areas have been identified for assistance.

  •  conducting a continuous professional development (CPD) needs analysis and connecting with local universities and education providers
  •  developing a wound management nurse led clinic, to provide training for clinic nurses in wound management
  •  develop a nurse led clinic model that can be replicated for emerging needs in the community that will enhance the clinical competencies of the practice nurse. Nurse led clinics are able to focus on local nurses and local problems and provide the most appropriate training and structured care.

This program provides for peer to peer support and nurturing of existing skills within the clinical workforce.

SWMML has a role to provide support and coordinate the learning of practice nurses to better understand the patients’ needs and the needs of the practice. This assists nurses to comply with the AHPRA requirements of 20 hours professional development.

Practice nurses are offered the opportunity to share best practice ideas and networking by attending workshops and events coordinated by SWMML.
