After Hours 

A key role for SWMML is to address the urgent after hours primary care needs of our community. 

Since July 2012 we have undertaken a number of projects to facilitate services and access to urgent after hours care

  • we have conducted a needs assessment of services in the area
  • we have promoted the after hours services available to the local community
  • we have funded extra nursing services to minimise unnecessary admissions to Emergency Departments from aged care facilities

Our focus has been to continue to support existing services that are working well and to enhance after hours services.

SWMML has contracted with general practices, Medical Deputising Services and other existing service providers to provide access to urgent after hours primary care services in our region. We are also providing an opportunity for grant incentive funding to those planning to extend their current service hours.

SWMML will directly fund Melbourne Medical Deputising Service (MMDS) and Australian Locum Medical Service (ALMS) to provide in home doctor visits after hours. By contracting with the major locum services in the area SWMML will be able to gain a more complete picture of the after hours needs in the area through a formal reporting criterion.

What does this mean for your practice?

Each practice in the Wyndham and Hobsons Bay area can choose either locum service provider and maintain the current clinical relationship. The combined Tier 1 payments will be used to fund this service and will be paid by SWMML.

SWMML will contract directly with those practices who previously received Tier 2 and Tier 3 payments.  This amount will be based on 2012-2013 payments while the practice continues to offer extended hours.

For those practices in the process of applying for PIP payments or who are planning to extend their opening hours SWMML will offer an opportunity to apply for an incentive grant.
